  Rate Information

There is no long term contract required. There is a minimum hours of service per visit which is four (4) hours. There is no minimum amount of service days per week required; you may choose to have service for one (1) or more days per week, depending on your need. Company has a billing cycle of every two (2) weeks. Payment is due upon immediately upon receipt.

There are NO overtime charges for utilizing the same caregiver for more than 40hrs in a 7 day work week.  The only time you will encounter an overtime charge (1 ½) times the normal service rate are on major holidays. Recognized holidays are as follows:

♦    New Year’s Day          ♦          Memorial Day              ♦          July 4th                        

♦    Labor Day                    ♦          Thanksgiving Day         ♦          Christmas Day

If a Horizon Nursing Service caregiver uses their own automobile on patient business (e.g taking client’s to doctors appointment, shopping of any errands) there is a mileage fee charged to client’s account that is paid in it’s entirety to the caregiver.

Cost for services varies depending on type of caregiver needed and level of care.  Actual cost can only be determined by contacting us with further information about your specific needs.

Forms of Payments Accepted

We accept: Most Long Term Care Insurance Policies, Personal checks, American Express, Visa and MasterCard.

Most Long Term Care Insurance Accepted

Horizon Nursing Service will accept assignment of insurance benefits as long as it covers home health care services. If allowed by your insurance company we will be happy to confirm the extent of your benefits. After obtaining a signed assignment of benefit from you, we will bill your insurance company directly and send you an invoice for portions not covered, if applicable. Because some insurance company will not allow us to verify your exact benefit information, we would like to give you some important tips when verifying your benefits.

1.      Remember to ask the representative if you have an elimination period.  An elimination period refers to a period of time that you the policyholder must pay out of pocket before the insurance company will start making payments.  Elimination periods vary depending on the benefits that the policyholder purchased.

2.      Ask the representative what is your daily benefit allowance.  This means what is the maximum dollar amount the policy will pay each day.

3.   Finally you want to find out what is the maximum dollar amount of your entire policy and for how many years will the policy be in effect for.
